Below is some information to help you have a smooth and efficient registration process:

Patient Portal Link

Please fill out the New Patient Registration forms prior to your visit via the portal link that you will receive via email.
The link is only valid for 24 hours. If you set up your login credentials within the 24-hour time frame, you can login to the Patient Portal afterwards to complete your forms. If you are unable to create your login credentials within 24 hours from the date of receiving the link, please let us know and we'll send you another one.


Registration Forms and Release of Records

Once you have successfully registered on the Patient Portal and to maximize you're scheduled appointment with our providers, please complete the following forms prior to your visit:

  • Patient Registration

  • Email Authorization

  • Office Policies

  • HIPAA Privacy Practices Document - This is at the end of the Patient Registration Form. When completing this form on the Patient Portal, you will be asked to click YES if you have read and received the complete consent form.   • Assignment of Benefits - Only needs to be completed by insured patients.

  • Release of Records - To ensure a more thorough clinical examination, please complete this form and we will request dental records from your current or previous provider.

Your answers on the registration form go directly to our clinical staff so we are better prepared for your visit.


Dental Insurance

Please provide insurance information if not provided during your initial call with us.

Our office protocol is to verify dental insurance prior to your upcoming appointment; therefore, we will need the following to send a PTE (pre-treatment estimate) to your insurance carrier to determine if your visit will be covered. Depending on the insurance carrier, these estimates can take up to 2-4 weeks to be processed. Please provide us with the following:

  • Subscribers complete name:

  • Subscriber's DOB:

  • Name of insurance carrier & STATE:

  • Subscriber/Member ID # or SS# :

  • Contact # for insurance carrier:


Getting here

If you are driving here, make sure you enter our physical address in your GPS/Phone: 410 Tremont Street, Boston, MA 02116.
Otherwise, you might be directed to another dental office that's also called Harmony near Downtown Crossing, that is also on Tremont Street.

We are located right next to the CVS/Pharmacy and directly across from the Animal Rescue League. (Map Link)


Parking

If driving to your appointment, here are some parking options near Harmony (Map link):

  • Metered parking on Tremont Street allows you to park for two hours. If you choose this option, try to arrive 10-15 minutes early in case you need a little extra time to find a spot.

  • Parking on Chandler Street (Map link) - This will probably be the best option given it's free two-hour parking and it's just a 1-2 minute walk from the office.

  • Whole Food's at Ink Block (Map link): Some of our patients choose to park at the Whole Food's on Harrison Avenue and do some shopping after their appointment. If you choose to do this, please note that Whole Foods has a two-hour parking limit.

  • Stanhope Garage, Inc. (Map link) - Located at the corner of Shawmut Avenue & E. Berkeley Street. This is about a 7-10 minute walk from the office.


Lastly, here is some information regarding Oral ID:

What is Oral ID?
Oral ID is an oral exam device that is designed to help us visualize oral cancer, pre-cancer and other abnormal lesions. The device uses fluorescence technology (a blue light) and is non-threatening to patients.

Why is it important?
Our patients oral and overall health are very important to us and we believe that early detection is critical when dealing with oral cancer issues.

Will my insurance cover it?
Oral cancer screening is an optional service we provide that is not covered by dental insurance companies. The cost of Oral ID is $30.
